A Winfield, Kansas daycare facility is shuttered following an investigation by state officials.

According to a spokesperson for the Kansas Department of Health and Environment, the closure was prompted by two surveys that turned up several code violations.

According to a department spokesperson, investigators found more than a dozen violations, including overflowing trash cans, dog feces on the bathroom floor, missing medical records for children and a missing background check of a worker.
